In a 5-quart slow cooker, pour in 2 cups heavy cream, cocoa powder, whole milk, and sweetened condensed milk. Whisk thoroughly to dissolve the cocoa powder and combine the liquids.

Add chopped semi-sweet baking bar and vanilla extract to the slow cooker. Stir to evenly distribute the chocolate.

Set slow cooker to low heat. Cook mixture for 3 hours, stirring every 20 minutes to help chocolate melt completely and prevent scorching.

Thirty minutes before serving, add the remaining 2 cups heavy cream. Stir gently to combine and continue cooking on low.
